Cortical linear lesions in Wernicke's encephalopathy: can diffusion-weighted imaging herald prognostic information?

چکیده
To the Editor: Typical clinical features of Wernicke’s encephalopathy are caused by vitamin B1 deficiency and include confusion, ataxia, and ophthalmoplegia. Although it can occur in all malnourished patients, it is far more frequent in alcoholics. Brain MRI is a central diagnostic tool that usually discloses high signal lesions in T2-weighted imaging symmetrically affecting mammillary bodies, the thalamus, hypothalamus, periaqueductal gray matter, midbrain, and the cerebellum. Cortical damage and diffusion-weighted imaging findings were both seldom reported in Wernicke’s encephalopathy.

Diffusion-Weighted MRI in Recurrent Wernicke's Encephalopathy: a Remarkable Cerebellar Lesion
We report unusual MRI findings (including those from diffusion-weighted imaging (DWI)) in a patient with recurrent Wernicke's encephalopathy with a remarkable cerebellar lesion. DWI showed high signal intensities in the superior portion of the cerebellar hemisphere and vermis area. After thiamine administration, clinical symptoms improved and the lesions with high signal intensities disappeared...
متن کاملMagnetic resonance imaging in the assessment of brain involvement in alcoholic and nonalcoholic Wernicke’s encephalopathy
AIM To present the typical and atypical magnetic resonance (MR) imaging findings of alcoholic and non-alcoholic Wernicke's encephalopathy. METHODS This study included 7 patients with Wernicke's encephalopathy (2 men, 5 women; mean age, 52.3 years) that underwent brain MR examination between January 2012 and March 2016 in a single institution. Three patients were alcoholics and 4 patients were...
متن کاملWernicke's encephalopathy: atypical manifestation at MR imaging.
SUMMARY We report a case of atypical manifestation of hyperintense lesions in a 64-year-old female patient with Wernicke's encephalopathy. Fluid-attenuated inversion recovery and T2-weighted images demonstrated symmetrical distribution of hyperintense lesions in cerebellar dentate nuclei, tegmentum of the lower pons, red nuclei, and tectum of the midbrain, and T1-weighted sagittal images showed...
متن کاملWernicke's encephalopathy with visual loss in a patient with hyperemesis gravidarum.
OBJECTIVE We describe a case of Wernicke's encephalopathy associated with visual loss that was caused by hyperemesis gravidarum. METHODS A 25 year old lady in her 20th week of her pregnancy consulted us. She had history of nausea and vomiting for 3 months with resultant weight loss. She now presented with sub-acute onset of visual loss in both her eyes, and gait disturbance with unsteadiness....
متن کاملWernicke's encephalopathy: a case report and MRI findings.
Wernicke's encephalopathy (WE) is a serious, potentially fatal acute or subacute neurological disorder caused by thiamine (Vitamin B1) deficiency. Although it is most frequently observed in patients who are chronic alcoholics, WE may also be associated with hyperemesis gravidarum. We report magnetic resonance imaging (MRI) and diffusion weighted imaging (DWI) findings in this case of WE in a pr...
